Overview & Objectives

The Polycare project aims to develop and test and integrated care model supported by advanced ICT systems. These systems and services will allow patients in acute phases to be monitored from home. This methodology strives to achieve the two main objectives:

  • Enhance quality of life and care for patients.
  • Increase sustainability of health and social care systems.

Better conditions of life and care for patients as well as sustainability improvement for health and social care systems.

site has been working on a sustainable integrated care business model for 6 years now. There is a strong technological background and well established infrastructure. However in Aragón, home hospitalization is a new concept for which there is growing interest  across the entire Aragonese region.
